Job summary

VNACare in California seeks a Hospice LVN to join our team in Rancho Cucamonga and surrounding areas on a per-diem basis. The role focuses on delivering compassionate hospice nursing care with flexible scheduling, including limited weekends and on-call shifts.

Ideal candidates will hold a CA LVN license with at least one year of acute-care experience and a preference for hospice and EMR familiarity. Mileage reimbursement and orientation/training are provided.

Qualifications

• Current LVN license in CA is required.
• Minimum one year of acute-care experience.
• Hospice experience preferred.
• EMR experience preferred.

Responsibilities

• Ensures quality and safe delivery of Hospice services.
• Implements current nursing practices and provide direct hands-on care within scope of practice.
• Obtains orders as needed to promote comfort and quality of life.
• Keep Case Manager informed of all changes in condition and orders obtained.

Requirements of the Hospice LVN:
