Job summary

VNAcare in Southern California is seeking a Hospice LVN for a per-diem position in Rancho Cucamonga. The role focuses on delivering compassionate hospice care with flexible scheduling and opportunities to work in a team-based environment.

Required CA LVN license and at least one year of acute-care experience; hospice experience and EMR familiarity are preferred to support quality patient care and coordination with case managers.

Qualifications

• Current LVN license in State of CA.

• Experience in hospice preferred.

Responsibilities

• Ensures quality and safe delivery of Hospice services.
• Implements current nursing practices and provide direct hands-on care within scope of practice.
