The Community Health Centers of Lane County is seeking a Family Nurse Practitioner to join an interdisciplinary team providing comprehensive primary care within the Patient Centered Medical Home model.
This position involves working at our West 11th Rapid Access Clinic, delivering professional, comprehensive primary medical care to a diverse patient population using a team-based approach. The Nurse Practitioner diagnoses and treats various health conditions, provides education and referrals, and performs related duties.
The clinician evaluates patients, assesses their medical status, prescribes medications, discusses case management with healthcare professionals, and documents all patient interactions according to policy. The position ranges from 0.8-1.0 FTE (32-40 hours weekly) and may include bilingual compensation upon passing a bilingual exam. All candidates are encouraged to apply.
The Community Health Centers of Lane County (CHCLC) has been providing primary care and behavioral health services since 2004 through eight clinics. Our care teams include primary care providers, nurses, pharmacists, medical assistants, and behavioral health specialists, focusing on integrated, patient-centered care that addresses social determinants of health.
We serve approximately 30,000 individuals annually, offering affordable care for all ages, including services like acupuncture and dental care. Our mission is to serve the community by eliminating healthcare barriers and promoting health and well-being.
Travel between clinic locations is required during onboarding and training. The schedule is Monday-Friday, 8 am-5 pm. This is an AFSCME Nurse represented position.
Equivalent experience and training demonstrating required knowledge and abilities are also acceptable. Specialty categories include: ANP, CNM, CHNP, FNP, GNP, NNP, PNP, PMHNP, WHCNP.
