Clinical Care Manager RN I (Health Plans) - Remote

This is a remote position available in specific states, with candidates residing out of state needing to work Pacific Time Zone hours.

Job Summary/Purpose

Assess, plan, implement, coordinate, monitor, and evaluate options and services to meet individual health needs. Apply knowledge of regulations, identify resources, eliminate barriers, and provide care management interventions across transitions of care for cases with low to moderate service intensity.

Qualifications
  • Current unencumbered Oregon RN License required; BSN preferred.
  • One (1) year clinical nursing experience required.
  • Experience or training in healthcare delivery systems and electronic documentation (e.g., MS Office, EPIC, Clinical Care Advanced).

Experience in case management and knowledge of Medicare and Medicaid rules are strongly preferred.

Skills and Abilities
  • Effective communication, confidentiality adherence, critical thinking, social perceptiveness, and knowledge of Medicare/Medicaid regulations.
Physical Demands

Includes activities like climbing stairs, lifting, bending, kneeling, standing, walking, reaching, pushing, pulling, sitting, manual dexterity, and fine motor skills, with varying frequency levels.
