Job summary

Pride Health is hiring a Nurse Practitioner in SC for a 6-month contract to hire. Schedule: Monday–Friday, 8am–5pm, on-site. Pay rate: $65–$70/hour.

You will provide specialized care to high-risk plan members, conduct comprehensive assessments, manage acute and chronic conditions, document with ICD-10, and collaborate with the care team to plan care and educate patients and caregivers. This role emphasizes patient education and coordinated care in a team-based setting.

Qualifications

• NP or PA licensure is required.
• NP certification in FNP, ANP, or GNP preferred.
• Access to transportation.
• 1+ year experience in LTC/SNF/Geriatrics preferred.
