Job summary

CMR Recruiting is seeking a Nurse Practitioner or Physician Assistant to serve as the primary care and rounding provider for a Skilled Nursing Facility in Chicago, IL. This full-time, in-person role runs Monday through Friday, 9 to 5, with no nights, weekends, or on-call; the last two hours may be completed remotely.

Candidates must hold an active Illinois NP or PA license with DEA, board certification in FNP/AGNP/AGPCNP or NCCPA for PA, and have experience in geriatrics or long-term care.

Qualifications

• Active and unrestricted Illinois NP/PA license required.
• DEA registration required.
• Board certification as FNP/AGNP/AGPCNP or NCCPA for PA.
• Experience in skilled nursing, long-term care or geriatrics preferred.

Responsibilities

• Provide primary care to residents including exams, assessments and medication management.
• Diagnose and treat chronic and acute conditions within scope of practice.
• Order and interpret labs, adjust care plans accordingly.
• Collaborate with facility nursing, therapy and interdisciplinary team.
• Maintain accurate and compliant clinical documentation.

Compensation: $115,000 - $145,000 Annual Salary

Position Description

You will serve as the primary care and rounding provider inside one skilled nursing facility in Chicago, IL, managing a panel of long term care and short stay rehab residents. This is a full time, in person position, Monday through Friday, 9 to 5, with no nights, no weekends and no on-call. The last two hours of the day can be completed remotely. You are employed by our provider network rather than the facility, so you do not report to the DON or the administrator, though you take part in patient care meetings.

Qualifications:

• Active and unrestricted Illinois NP or PA license and DEA registration
• Board certification as an FNP, AGNP or AGPCNP, or NCCPA certification for a PA
• Experience in skilled nursing, long term care or geriatrics preferred
• Comfortable managing a full resident panel independently
