Qualifications

• Board certification in family, adult-geriatric, or related field.
• DEA licensure.
• Clinical experience in adult, family or geriatric care.
• Experience in long-term care setting.

Responsibilities

• Provide primary and preventive care for long-term care residents.
• Manage residents with acute changes in condition.
• Engage in advanced care planning and chronic care management.
• Coordinate with nursing home staff and primary physician.

Job Description: Our Skilled Nursing Facility in Toledo, OH is looking for a Nurse Practitioner to join a facility-based team of medical providers caring for patients living in long-term care facilities.

• Manage residents with an acute change in condition
• Advanced care planning and chronic care management
• Order, review and follow up on treatments and diagnostic testing
• Communicate regularly with nursing home staff and collaborate with the primary care physician
