Job summary

American Physiatry seeks a compassionate Nurse Practitioner to join our healthcare team, focusing on pain control and functional improvement in skilled nursing facilities. You will work closely with a Physiatrist, conduct patient assessments, develop care plans, and educate families on treatment plans and preventive care.

Ideal candidates have CT licensure, the ability to manage a caseload independently, and a commitment to high-quality, collaborative patient care in a fast-paced setting.

Qualifications

  • Ability to complete clinical documentation promptly in compliance with policies

Responsibilities

  • Assess patients and manage conditions under supervision of a Physiatrist.
  • Develop and implement individualized care plans to improve function and rehabilitation.
  • Collaborate with healthcare team on medications and therapy recommendations.
  • Educate patients and families on treatment plans, health maintenance, and preventive care.
  • Monitor progress and adjust care plans to achieve optimal outcomes.

Key Responsibilities

  • Conduct thorough assessments of patients, identifying and managing medical conditions under the supervision of a Physiatrist.
  • Develop and implement individualized care plans, focusing on function and rehabilitation to improve patients' quality of life.
  • Collaborate with the healthcare team to ensure comprehensive care, including medication management and therapy recommendations.
  • Educate patients and their families on treatment plans, health maintenance, and preventive care.
  • Monitor and evaluate patient progress, adjusting care plans as necessary to achieve optimal outcomes.

Qualifications

  • Licensed Nurse Practitioner with a valid Connecticut certification.
  • Experience working independently while effectively managing your own caseload.
  • Submit accurate and thorough clinical documentation promptly, ensuring compliance with company policies and regulatory requirements.
