Job summary

VITAS Healthcare in Colorado is seeking a Nurse Practitioner to join our hospice care team. You will assess patients, develop care plans, and collaborate with physicians and the interdisciplinary team to ensure compassionate end-of-life care.

The role requires active NP licensure, completion of an accredited NP program, and a minimum of two years in hospice, home health, or community health. This position involves routine mobility tasks and interactions with patients and families.

Qualifications

• Minimum 2 years experience in hospice, home health or community health in the past 5 years.
• Physically capable of lifting and transferring patients; lift in excess of 20 pounds.
• Stand or walk >1 hour per day; talk, hear, sit, stoop, kneel and crouch as needed.

Responsibilities

• Records physical findings and formulates care plan and prognosis.
• Discuses case with physician to prepare comprehensive care plan.
• Prescribes or recommends drugs or other treatments.
• Monitors patient progress and reports to supervising physician.
• Collaborates with hospice employees and volunteers as part of an IDT.

• Monitors and maintains progress of patients and reports progress to supervising physician.
• Has the ability to work collaboratively with hospice employees and volunteers as part of an IDT.

QUALIFICATIONS

• Qualified candidates must have a minimum of (2) years experience in hospice, home health or community health in the past five (5) years.
• While performing the duties of this position, the employee is regularly required to assist in lifting and transferring patients as required.
• Lift objects in excess of 20 pounds.
• Specific vision abilities required by this job include: close vision, distance vision and ability to adjust focus.
• The employee will be required to stand or walk in excess of one hour per day, talk, hear, sit, stoop, kneel and crouch as needed.

EDUCATION

• Requires completion of accredited Nurse Practitioner program.
• State licensure is also required.
