Overview

The Team Manager is the leader of the Patient Care Team and is responsible for supervising, evaluating and coordinating the interdisciplinary team; assuring continuity of care from admission to discharge or transfer to bereavement; serving as patient advocate and coordinating with other social service and health care providers involved in the care of the team’s patients; and maintaining patient records from intake through discharge or transfer to bereavement.

Responsibilities
  • Direct all patient care services for the team, including volunteer and bereavement, and ensure that services are rendered as integrated components of the interdisciplinary plan of care.
  • Review information on pending admissions, new admissions (assessments and history), and “alive” discharges to assure eligibility and appropriateness criteria are met.
  • Collaborate with the team physician and the attending physician regarding palliative care measures for pain and symptom management.
  • Ensure complete and accurate data and documentation are available to make recertification decisions and that those decisions are timely.
  • Review imminent “alive” discharges to assure effective discharge planning.
  • Monitor the type and level of team services provided (staffing, medications, HME, supplies, level of care changes) to ensure they are appropriate and responsive to patient/family needs and expectations.
  • Manage the team schedule and coordinate services 24/7 for all patients on the team.
  • Facilitate team meetings with interdisciplinary input into updates and revisions of the plan of care in line with changing patient and family needs.
  • Participate in on-call rotation.
Staff Supervision and Management
  • Interview, select, train, supervise, evaluate and dismiss team staff in conjunction with the Patient Care Administrator.
  • Ensure staff competence and performance through field visits, review of documentation and team-based in-services on VITAS values, hospice principles and practices and team growth needs.
  • Act as a resource and mentor for staff on clinical issues, documentation, problem solving, and customer service behavior.
  • Oversee staff and volunteer schedules and territory assignments to distribute workload equitably and meet productivity expectations.
  • Review and approve payroll with accurate time, mileage and expenses.
Quality Improvement/Regulatory Compliance
  • Monitor clinical records and team activities to ensure accuracy, completeness and compliance with VITAS policy and regulatory bodies.
  • Develop and implement performance improvement activities in response to service issues and participate in quality and service improvement programs.
  • Ensure completion of proactive patient/family satisfaction surveys and implement Quality Improvement measures.
  • Assist with regulatory surveys and requests as directed by local program.
  • Perform substantive chart reviews to ensure evidence of quality care and participate in outcomes management and annual program review.
  • Perform utilization review of continuous care and inpatient levels of care for all patients on the team.
Customer Service / Marketing
  • Address problems, grievances, or service failures for patients, families or physicians/MCOs and resolve them promptly.
  • Analyze customer service issues to identify causes and work with the team to improve performance.
  • Speak with patients/families and attending physicians when patient revocation is considered to seek satisfactory solutions to avoid unnecessary revocation.
  • Regularly visit LTC and Contract Bed facilities to ensure care plan integration and customer satisfaction, and discuss criteria for determining appropriateness.
  • Provide in-services to customers with team members and program staff.
  • Participate in professional, voluntary or community service organizations.
Cost Containment
  • Verify/approve admitting IPOC diagnoses, treatments, staffing and supplies.
  • Monitor resource utilization for cost-effective service delivery.
  • Control and be accountable for productivity, labor and all related patient care costs within the budget.
  • Approve all bills/invoices related to patient care services.
Professional Development
  • Attend in-services, educational seminars and workshops.
  • Develop and achieve professional growth goals and objectives.
  • Mentor newly hired Team Managers.
Qualifications
  • Reliable transportation with appropriate license and insurance for driver and passengers.
  • Private telephone in home.
  • Knowledge of principles and practices of primary medical and nursing care, with working knowledge of oncologic nursing and palliative care.
  • Understanding of interpersonal dynamics within an interdisciplinary health care team and knowledge of the process of dying and bereavement.
  • Working knowledge of VITAS information system (Vx) and ability to utilize management reports.
  • Two years of supervisory experience or equivalent in a health care organization.
Education
  • Bachelor’s degree preferred.
  • Current and valid RN license to practice in the state where the VITAS program is located.
