Job summary

VITAS Healthcare is seeking a part-time Nurse Practitioner to join our hospice program in Port Saint Lucie, FL. You will record physical findings and formulate care plans, while collaborating with physicians to create comprehensive patient care plans and monitor patient progress.

The role requires an accredited NP program completion and state licensure, with at least 2 years of hospice/home health experience. You will work with a dedicated IDT to provide quality end-of-life care.

Qualifications

• Minimum of 2 years experience in hospice, home health or community health in the past five years.
• Must be able to assist in lifting and transferring patients as required.
• Lift objects in excess of 20 pounds.
• Specific vision abilities: close vision, distance vision and ability to adjust focus.
• Stand or walk in excess of one hour per day; talk, hear, sit, stoop, kneel and crouch as needed.
