Responsibilities

• Monitor healing progress and adjust treatment plans as needed.
• Stay updated with advances in wound care and evidence-based practices.
• Go to patients' homes and facilities to provide proper orders.

Position Overview:

Responsibilities:

Conduct comprehensive wound assessments and develop individualized treatment plans for patients with acute and chronic wounds

Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated and effective wound care

Provide education and counseling to patients and their families on wound prevention, care techniques, and self-management

Monitor and evaluate the progress of wound healing, adjusting treatment plans as necessary

Utilize evidence-based practices and stay current with advancements in wound care.

Go to patients homes and facilities and work with HH and facility staff to give proper orders

Requirements:

• Current state NP license and certification

• Travel will be required to provide onsite and mobile wound care treatment

Experience:

• Advanced Wound Care; grafting; debridement: - Required

Preferred Qualifications:

• Certification in wound care (e.g., Wound, Ostomy, and Continence Nursing Certification)
• Experience with electronic health records (EHR) systems
• Knowledge of the latest wound care products and technologies
