Job summary

PrimeHealth+ is seeking a Graduate Degree APN/NP to join our outpatient clinic in Grand Junction, CO. This full-time, in-person role focuses on comprehensive primary care with a collaborative team approach.

The position offers a four-day workweek, integration with Behavioral Health, on-site pharmacy and psychiatry services, and a strong emphasis on wellness and chronic disease management. Sign-on bonus and loan-repayment eligibility are available.

Qualifications

• Master’s degree or higher in APN/NP program.
• Colorado license to practice as APN/NP and DEA license to prescribe.
• Life Support (BLS) certification at minimum.
• Ongoing Continuing Education as required by credentialing boards.

Responsibilities

• Provide comprehensive outpatient primary care including wellness visits and chronic disease management.
• Collaborate with Behavioral Health providers across clinic pods.
• Work with a robust team of MAs, RNs, patient navigators, case managers and care coordinators.
• Opportunity for in-office procedures based on interests.

QualificationsRequired Education/Experience:

• Master’s degree, equivalent education, or higher degree from an accredited APN/NP program is required.
• Holds and maintains without restrictions a Colorado license to practice as an APN/NP, DEA license to prescribe medication*, Life Support certification (BLS at minimum).
• *if the APN/NP has provisional prescriptive authority requiring an articulated plan it is the responsibility of the APN/NP to alert the CMO and Quality director of such and to actively participate in meeting related requirements.
• Maintenance of Continuing Education as required by your APN/NP credentialing board and state medical board.

Preferred Education/ Experience:

• Two years of related experience as an APN/NP
