Job summary
HopeWest in Grand Junction, CO is seeking a PACE Nurse Practitioner to provide primary care, case management, and care coordination for a panel of participants, under state and federal guidelines. The NP will collaborate with physicians and specialists to develop the plan of care, focus on prevention, wellness, and management of acute and chronic conditions, and ensure timely documentation in the EMR.
A current Colorado APN license, DEA license, and MS Office/EMR skills are required.
Qualifications
- Board-certified Advanced Practice Nurse licensed in Colorado (APN) and RXN; Colorado license required.
- DEA license with valid number.
- CPR certification maintained.
- Experience with EMR and MS Office.
Responsibilities
- Provide primary care and case management for assigned participant panel.
- Develop and update Plan of Care (POC) in collaboration with the care team.
- Coordinate with physicians and specialists to meet POC goals and ensure timely care.
