Job summary

PrimeHealth+ seeks a Primary Care APP for outpatient, in-person care in Grand Junction, CO. The role offers a 4-day workweek, a no-call policy, and integration with behavioral health services across two primary care clinics and two school-based centers.

The team includes MAs, RNs, care coordinators, and on-site pharmacy. Competitive pay, benefits, sign-on, and NHSC/CHSC loan repayment eligibility are offered.

Qualifications

• Master’s degree or equivalent education from an accredited PA program.
• Colorado license to practice as a PA (unrestricted).
• DEA license to prescribe medication.

Responsibilities

• Provide outpatient primary care services within an integrated care team.
• Collaborate with Behavioral Health and other specialists on clinic pods.
• Participate in weekly Lunch&Learns and ongoing professional development.
• Support comprehensive preventive and chronic disease management.

• Position is full-time, in-person

QualificationsRequired Education/Experience:

• Master’s degree or equivalent education from an accredited PA program is required.
• Two years of related experience as a PA is preferred
• Holds and maintains without restrictions a Colorado license to practice as a PA, DEA license to prescribe medication, Life Support certification (BLS at minimum).
• Maintenance of Continuing Education as required by your PA credentialing board and state medical board.

Preferred Education/Experience:

Two years of related experience as a PA
