Job summary

Carease Health in Carlyle, Illinois is seeking a Nurse Practitioner to deliver high-quality preventative care to residents in skilled nursing facilities, including on-site visits and remote work from home. You will assess, plan treatments, prescribe as needed, and collaborate with healthcare teams to reduce hospitalizations while utilizing RPM, CCM, and CoCM to improve patient outcomes.

This is a hybrid role with flexible scheduling and strong leadership support.

Qualifications

• Active NP license in the state of practice.
• 1+ year of geriatric care and documentation review experience.
• Excellent communication, collaboration, and teamwork skills.

Responsibilities

• Provide high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote work from home.
• Conduct assessments, develop treatment plans, and prescribe medications as needed.
• Collaborate with healthcare teams to ensure comprehensive care and minimize hospitalizations.
• Utilize RPM, CCM, and CoCM to enhance patient outcomes.
• Maintain accurate documentation of patient care and outcomes.
• Participate in quality improvement initiatives to enhance patient care standards.
