You will collaborate with clinical directors, regional managers, and supervisors to maintain high-quality care, focusing on geriatrics with an emphasis on patient-centered outcomes.

Qualifications

• Active NP or PA license in the state of practice.
• 1+ year of experience in geriatric care and documentation review.
• Excellent communication, collaboration, and teamwork skills.

As a Nurse Practitioner with Carease Health you will play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. We're seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to join our team.

About the Role:

As a Nurse Practitioner with Carease Health you will:
