Job summary

Wilmington Community Clinic in California is seeking a Nurse Practitioner to deliver direct primary care across the lifespan, diagnosing, planning, and coordinating care as part of a collaborative team. You will work with physicians and the care team to provide high-quality, patient-centered services in a community-focused setting.

Ideal candidates hold MSN or DNP, California NP and furnishing licenses, active NPI, and be proficient with EPIC.

Qualifications

  • Master's or Doctoral degree in Nursing (MSN/DNP) with NP certification.
  • Valid NP license and California furnishing license.
  • Active NPI, current BLS/CPR, and applicable DEA registration.

Responsibilities

  • Provide direct primary care services across lifespan, including diagnosis and treatment planning.
  • Collaborate with physicians and care team to deliver patient-centered care.
  • Coordinate care and navigate patients to WCC and community resources.
  • Develop and implement individualized care plans for acute and chronic conditions.
  • Perform orders and interpret diagnostic tests; prescribe within scope.

Position Summary

The Nurse Practitioner provides direct primary care services to patients across the lifespan, including diagnosis, treatment planning, and coordination of care. Works collaboratively with physicians and the care team to deliver high-quality, patient-centered care, and serves as a key clinical resource within Wilmington Community Clinic's primary care team.

Benefits

The compensation for this position is $130,000.00 - $145,000.00 annually. Education and Experience
  • Master's or Doctoral degree in Nursing (MSN/DNP) from an accredited college or university, with certification as a Nurse Practitioner.
  • Valid Nurse Practitioner license and Furnishing License issued by the California Board of Registered Nursing.
  • Active National Provider Identifier (NPI), current BLS/CPR certification, and DEA registration as applicable to role.
  • Provides comprehensive primary care services, including history-taking, physical examination, diagnosis, and treatment planning for patients across the lifespan.
  • Orders, performs, and interprets diagnostic tests; prescribes medications and treatments within scope of practice and California furnishing authority.
  • Develops and implements individualized care plans for patients with acute and chronic health conditions.
  • Provides preventive care, health education, and counseling to patients, families, and caregivers.
  • Assists patients in navigating their care by connecting them with appropriate WCC and community resources as needed.
  • Assists with administration, development, implementation, and evaluation of clinical programs and protocols.
  • Collaborates with physicians in reviewing and co-signing charts, as required under California furnishing regulations.
  • Knowledge of computer operations; ability to readily adapt to the EPIC electronic health record system.
  • Follows clinic policies and procedures to ensure that the principles of WCC are implemented.
  • Fosters an environment that promotes trust and cooperation among all staff of WCC.
  • Attends all WCC mandatory meetings and other meetings as requested.
  • Interacts with patients, physicians, staff, vendors, and visitors in ways that demonstrate caring and reflect the WCC mission and philosophy.
  • Completes other duties as assigned in support of WCC's mission.

  • Maintain computer skills adequate to perform EPIC duties within 90 days of employment.
  • Knowledgeable of clinical guidelines, evidence-based practice standards, and primary care procedures and resources.
  • Ability to communicate effectively.
  • Punctuality and excellent attendance required.
