Job summary

A healthcare provider organization is seeking Nurse Practitioners in Montana to conduct in-home health assessments. This role offers flexibility to design your own schedule and work autonomously in the field. Nurse Practitioners will perform consultative evaluations for Medicare patients, focusing on preventive care and identifying potential gaps in health services. Candidates must be state licensed and board certified, with preference given to those with experience in wellness exams. Comprehensive support is provided, including training and scheduling tools.

Qualifications

• Must be State licensed and Board Certified Nurse Practitioner with NPI.
• Prefer at least 6 months of experience in doing wellness exams.

Responsibilities

Annual Wellness Assessment Nurse Practitioner. Nurse Practitioners (NP) needed to conduct In-Home Health Assessments in many locations throughout Montana. We offer year-round opportunities: Full-time, and Part-time

Set your own schedule and earn an excellent income! This is an autonomous field-based, prevention/education-focused opportunity that takes you out of the mundane office setting.

Our healthcare providers conduct comprehensive, in-home Health/Annual Wellness Visits with Medicare patients as part of their health insurance benefits. The evaluations are conducted in a face-to-face manner where the member resides. Within the visit, the clinician reinforces the importance of staying current on covered preventive services while identifying potential gaps in care, which stem from unnecessary hospitalizations and ER visits.

• Flexibility - Design your own schedule with weekend visits as an option
• No prescribing or treatment, simply performing consultative assessments/evaluations

Requirement:

• Must be State licensed and Board Certified Nurse Practitioner with NPI.
• Prefer at least 6 months of experience in doing wellness exams.
