A healthcare organization is seeking Nurse Practitioners to conduct in-home and embedded clinic health assessments. This position offers the flexibility to set your own schedule, with full-time or part-time opportunities available. Responsibilities involve performing evaluations for Medicare patients, focusing on preventive services without prescribing treatments. Candidates must be licensed and certified NPs, preferably with experience in wellness exams. Comprehensive support, including credentialing and training, will be provided.

Qualifications

• Must have a Nurse Practitioner license and certification.
• At least 6 months of experience in conducting wellness exams.

Responsibilities

Wellness Visit Nurse Practitioner. Nurse Practitioners (NP) needed to conduct In-Home and embedded clinic Health Assessments. (Many locations) We offer year-round opportunities: Full-time or Part-Time.

Set your own schedule and earn an excellent income! Our healthcare providers conduct comprehensive, in-home, telemedicine, and mobile Health/Annual Wellness Visits with Medicare patients as part of their health insurance benefits. The evaluations are conducted in a face-to-face manner where the member resides as well as PCP clinic. Within the visit, the clinician reinforces the importance of staying current on covered preventive services while identifying potential gaps in care, which stem from unnecessary hospitalizations and ER visits.

• Flexibility - Design your own schedule with weekend visits as an option
• No prescribing or treatment, simply performing consultative assessments/evaluations

Requirement:

• Must be State licensed and Board Certified Nurse Practitioner with NPI.
• Prefer at least 6 months of experience in doing wellness exams.

• Scheduling of appointments with Medicare members within a certain time frame.
• Web tracking and scheduling tools to maximize your travel time.
