Job summary

Bristol Hospice Bakersfield is seeking a compassionate Nurse Practitioner to deliver hospice care to adults and children. You will manage terminal illness with a physician supervisor, perform comprehensive evaluations, and develop individualized care plans to support patients and families with dignity and comfort.

Qualifications

• Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program.
• Must have current nursing licensure in state.
• Must have a master's degree with a minimum of one year NP experience and hospice/palliative care experience.
• Must have accreditation from an approved certifying body for advanced practice nursing.

Responsibilities

• Provide medical care, palliate and manage terminal illness under physician supervision for adults and children.
• Complete history and physical examinations and document findings.
• Develop an individualized plan of care and coordinate hospice services.
• Complete hospice face-to-face visit requirements to assess eligibility.
• Provide clinical information to the hospice medical director for eligibility decisions.
• Make recommendations on eligibility for hospice certification/recertification.
• Review and coordinate patient care with the interdisciplinary team.
• Ensure patient comfort and symptom control through team collaboration.
• Review protocols and propose treatment options based on literature.
• Consult the physician when plans fall outside standard practice.
• Collaborate with the interdisciplinary group to update the plan of care.
• Fulfill assignments and communicate end-of-life support to patient/family.
• Complete all other duties as assigned.

Qualifications

On an Average Day You Will:

• Provide medical care, palliate and manage terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician to adults and children
• Complete history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record
• Manage health conditions and terminal illness by developing an individualized plan ofcare, prescribing medications and/or treatments, obtaining consultations or makingreferrals and coordinating hospice care services
• Complete the hospice face to face visit requirements prior to the third and subsequenthospice benefit periods to assess hospice eligibility based on the hospice conditions ofparticipation and the local coverage determinations
• Provide clinical information to the hospice medical director for interpretation of eligibilityfor hospice recertification
• Make recommendations to the hospice medical director regarding patient eligibility forcertification and recertification for hospice care
• Review clinical information, coordinate and oversee patient care in consultation with thehospice interdisciplinary team and other healthcare practitioners
• Assure that patient receives appropriate measures to control symptoms, throughcollaboration with interdisciplinary team members
• Review and develop protocols for treatment and propose options for interventionsbased on the literature in collaboration with the collaborating physician
• Consult the collaborating physician when the patient's plan of care is outsidestandardized practice and protocols
• Collaborate with the interdisciplinary group to create, review and revise the patient'splan of care
• Fulfill the obligation of requested and/or accepted assignments
• Demonstrate knowledge in communication and counseling patient/family in dealing withend-of-life issues
• Complete all other duties as assigned

Requirements

• Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program
• Must have current nursing licensure in State
• Must have a master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience
• Must have accreditation from an approved certifying body for advanced practice nursing as required by the State
• Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics)
• Must be a licensed driver with licensed automobile that is insured in accordance with organization/state requirements and is in good working order
