Job summary

Bristol Hospice is seeking a compassionate Nurse Practitioner to join our Greater LA team in Lakewood, CA. This PRN position focuses on hospice care, providing comfort, dignity, and collaborative support to patients and families at the end of life.

The NP will operate under physician supervision, manage terminal illness care, and coordinate with an interdisciplinary team to develop individualized care plans.

Qualifications

• Graduate of an accredited School of Nursing and NP program.
• Current nursing licensure in the State.
• Master's degree with a minimum of one year NP experience and one year hospice or palliative care experience.
• Accreditation from an approved certifying body for advanced practice nursing as required by the State.
• Certification in a specialist area preferred (e.g., CHPN, Pediatrics or Geriatrics).
• Must be a licensed driver with a insured vehicle in accordance with requirements.

Responsibilities

• Provide medical care and manage terminal illness per state Nurse Practice Act under physician supervision for adults and children.
• Perform history, physical exams, order/interpret lab tests, diagnose, and document findings.
• Develop individualized plans of care, prescribe, obtain consultations, and coordinate hospice services.
• Complete hospice face-to-face visits for eligibility assessments.
• Provide clinical information to hospice medical director for eligibility decisions.
• Collaborate with the hospice interdisciplinary team to coordinate patient care.
• Review and develop treatment protocols; propose intervention options based on literature.
• Consult the physician when the plan is outside standard practice.
• Ensure appropriate symptom control through team collaboration.
• Fulfill assignments and support end-of-life counseling for patients/families.

Responsibilities

• Provide medical care, palliate and manage terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician to adults and children
• Complete history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documenting the findings in the patient's medical record
• Manage health conditions and terminal illness by developing an individualized plan of care, prescribing medications and/or treatments, obtaining consultations or making referrals, and coordinating hospice care services
• Complete the hospice face‑to‑face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations
• Provide clinical information to the hospice medical director for interpretation of eligibility for hospice recertification
• Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care
• Review clinical information, coordinate and oversee patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners
• Assure that the patient receives appropriate measures to control symptoms, through collaboration with interdisciplinary team members
• Review and develop protocols for treatment and propose options for interventions based on the literature in collaboration with the collaborating physician
• Consult the collaborating physician when the patient's plan of care is outside standardized practice and protocols
• Collaborate with the interdisciplinary group to create, review and revise the patient's plan of care
• Fulfill the obligation of requested and/or accepted assignments
• Demonstrate knowledge in communication and counseling patients/families in dealing with end‑of‑life issues
• Complete all other duties as assigned

Requirements

• Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program
• Must have current nursing licensure in the State
• Must have a master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience
• Must have accreditation from an approved certifying body for advanced practice nursing as required by the State
• Certification in a specialist area preferred (e.g., Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics)
• Must be a licensed driver with a licensed automobile that is insured in accordance with organization/state requirements and is in good working order
