Responsibilities:

• Conduct comprehensive patient assessments, physical exams, and health screenings
• Diagnose and treat acute and chronic illnesses in a family medicine setting
• Develop and manage individualized patient care plans
• Prescribe medications and therapies in accordance with state regulations
• Order and interpret diagnostic tests including labs and imaging
• Provide preventive care, health education, and wellness counseling to patients and families
• Collaborate with physicians, specialists, and care team members to ensure coordinated patient care
• Maintain accurate and timely documentation in the electronic health record (EHR) system

Requirements:

• Active Nurse Practitioner (NP) license in the state of California
• Master's degree or Doctorate in Nursing Practice (MSN or DNP) from an accredited program
• Current DEA certification and ability to prescribe medications
• Board certification through AANP or ANCC required
• Experience in family medicine, primary care, or a related clinical setting preferred
• Strong bilingual communication skills (English/Spanish) a plus
• Compassionate, patient-focused demeanor with excellent interpersonal skills
• Proficiency with EHR systems and clinical documentation
