Job summary

Maria Parham Health in Henderson, NC is seeking a Nurse Navigator (RN) for the Heart and Vascular Department. This full-time daytime role supports patient coordination, care planning, and collaboration with physicians and staff to ensure seamless vascular care.

The RN will assess patients, develop and monitor nursing care plans, intervene as needed, and guide adults and geriatrics through the continuum of care. A NC RN license and two years of experience are required.

Qualifications

• Requires NC RN license (or compact) and current licensure in good standing.
• Two years of RN experience is required.
• BLS certification; ACLS to be obtained within 6 months.

Maria Parham Health provides care that addresses the many health issues of patients with vascular disease. Maria Parham Health's multidisciplinary team provides care that addresses the complexity of vascular disease and offers an array of services ranging from medical and surgical management of vascular disease, vein and wound care to peripheral vascular intervention.

Summary:

Assures optimum quality patient care by serving as a cardiac navigator resource liaison for all ancillary departments and medical staff members.

Reports to:

Director of Physician Services

Essential Functions:

• Performs assessment skills.
• Plans nursing care.
• Intervenes with appropriate action after needs are identified.
• Evaluates the assessment, planning and intervention process.
• Demonstrates professional and personal development.
• Delivers direct patient care appropriate to the age of patient served including adult and geriatric patients.

Additional Skills:

• Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
• Must be able to work in a stressful environment and take appropriate action.
• Requires excellent organizational skills and knowledge regarding the cardiac patient population to effectively identify the patients' needs and address them in a timely manner.
• Must have good communication skills with patients, family and or caregivers.
• Collaborates well with all physicians and coworkers.

What we're looking for

• Current Registered Nurse (RN) license in the state of North Carolina (or compact license)
• Bachelor of Science in Nursing (BSN)(preferred)
• Basic Life Support (BLS); Advanced Cardiovascular Life Support (ACLS) - to be obtained within 6 months
• Two years of RN experience(required)
• Active North Carolina RN license (single state) or compact required. If relocating, licensure must be updated to the new Primary State of Residence (PSOR) before employment begins
