Formación

• RN license/certification required upon hire
• BLS certification required upon hire
• Graduate of an accredited Nursing program
• Bachelor's Degree in Nursing preferred

Responsabilidades

• Coordinate patient-centered care across the continuum of services.
• Educate patients and families on care plans and available services.
• Assess patient needs and barriers to care; develop navigation plans.
• Track outcomes and support quality-improvement initiatives.
• Advocate for patients and liaise with care teams and community resources.
• Foster interdisciplinary collaboration for continuity of care.
