Position Summary
Under general administrative direction, coordinate and manage treatment plans for patients with different medical conditions; provide a significant level of patient education related to illness and planned management and treatment. This role involves overseeing the INR (International Normalized Ratio) Clinic, Inclisiran injection clinic, facilitating communication among multidisciplinary teams, coordinating patient care, ordering necessary labs and tests, and assisting providers in addressing any clinical insufficiencies. The Navigator serves as a central point of contact for patients, ensuring continuity of care and optimizing patient outcomes.
