Description:

POSITION PURPOSE:

The Nurse Health Navigator will serve on the front lines of one of our most integral programs, helping adults with serious and persistent mental illnesses/emotional disorders. The role acts as a critical liaison between healthcare providers, patients, and community resources. It focuses on improving patient outcomes by guiding individuals through the healthcare system, addressing social determinants of health, and connecting them with community-based support services.

WHAT YOU WILL DO:

  • Assess the needs of persons served and develop individualized care plans.
  • Provide education on chronic disease management, preventive care, and available community resources.
  • Collaborate with physicians, social workers, and other healthcare professionals to coordinate care and ensure smooth transitions between healthcare settings.
  • Assist persons served in scheduling appointments, understanding treatment plans, and overcoming barriers to care.
  • Serve as the primary departmental liaison with CCBHO and the Service Coordination Unit for the Wellness initiative.
  • Provide education to the Service Coordination unit and clients on Wellness metrics and other relevant topics as needed.
  • Track clients meeting the metrics set by CCBHO for Wellness, and document these metrics for both the assigned Service Coordinator and CCBHO.
  • Use clinical judgment to initiate services, schedule appointments, develop community support systems, and ensure educational goals are met for clients, in collaboration with the Service Coordinator.

MINIMUM QUALIFICATIONS:

  • BSN degree with 1 year of nursing experience OR Associate RN Degree with 2 years of nursing experience.
  • Psychiatric, community-based nursing experience is strongly preferred.
  • Current valid RN license in Pennsylvania.
  • Act 33/34 and FBI clearances.
  • Valid driver’s license and ability to drive an organization vehicle.
  • Pre-employment drug screening.
  • Certified Drug and Alcohol Counselor certification is desirable.

HIGHLIGHTS/BENEFITS

WORK SCHEDULE: This is an Exempt (salary) position. Monday-Friday, 9:00 AM - 5:00 PM, with flexibility for consumer needs. Some non-traditional hours (evenings, weekends, holidays).
