POSITION PURPOSE:
The Nurse Health Navigator will serve on the front lines of one of our most integral programs, helping adults with serious and persistent mental illnesses/emotional disorders. The role acts as a critical liaison between healthcare providers, patients, and community resources. It focuses on improving patient outcomes by guiding individuals through the healthcare system, addressing social determinants of health, and connecting them with community-based support services.

WORK SCHEDULE: This is an Exempt (salary) position. Monday-Friday, 9:00 AM - 5:00 PM, with flexibility for consumer needs. Some non-traditional hours (evenings, weekends, holidays).
