Job summary
University of Iowa Health Care in Iowa City is seeking a Nurse Clinician/Nurse Navigator to coordinate patient transitions and care across the continuum. You will work with an interdisciplinary team to plan discharges, optimize resource use, and ensure high-quality outcomes.

Qualifications
- Baccalaureate degree in Nursing is required.
- Current license to practice nursing in Iowa or ability to obtain by hire date.
- Experience working with an interdisciplinary team and strong communication skills.
Responsibilities
- Perform transition planning activities and create personalized transitional care plans.
- Complete initial assessments to identify barriers impacting length of stay and discharge planning.
- Collaborate with providers and care team to manage length of stay and determine appropriate level of care.
- Assist in discharge planning and referrals for post-acute services.
- Communicate care plans to the health care team and patient/caregiver; ensure plan adherence.
