Job summary

The University of Iowa Health Care’s Care Coordination Division seeks a Nurse Clinician/Nurse Navigator to serve as a clinical nurse expert for the Inpatient Burn Treatment Unit and to coordinate care for the assigned patient population, facilitating seamless transitions across the continuum of care.

The role includes clinical assessment, education reinforcement, follow-up, and collaboration with an interdisciplinary team to optimize quality health care delivery and resource utilization.

Qualifications

• Minimum of 3 years of recent clinical nursing experience.
• Current license to practice nursing in Iowa, or ability to obtain licensure by the date of hire.
• Experience working with an interdisciplinary team.
• Excellent organizational, critical thinking, problem-solving, and communication skills.
• Proficiency in computer applications including Microsoft Excel, Word, and PowerPoint.

Responsibilities

• Perform transition planning activities and create/update personalized transitional care plans.
• Complete initial assessments to identify barriers to discharge and resource needs.
• Collaborate with providers and care team to manage length of stay and care transitions.
• Assist in discharge planning for post-acute services.
• Articulate and communicate the plan of care to the care team and patient/caregiver.
• Intervene to maintain care progression and address deviations in the plan.
• Participate in multidisciplinary huddles and share plan details with the health care team.
