Job summary

Allegheny Health Network is seeking a RN Nurse Navigator to serve as the primary contact for referring physicians, patients, families, and community members. You will coordinate patient flow, including scheduling, and help patients access clinical and supportive care services within the network.

The navigator collaborates with a multidisciplinary team to ensure quality and comprehensive services for adult patients.

Qualifications

• Current PA RN licensure or multi-state licensure through eNLC.
• 1-3 years nursing experience in the specialty required.
• Bachelor's Degree in Nursing preferred.
• Certification in area of expertise.

Responsibilities

• Oversees community outreach and coordinates care across the network.
• Educates patients and families about diagnosis, treatment options, and care plans.
• Coordinates appointments and handoffs between facilities and disciplines.
• Monitors patient outcomes and supports care transitions (inpatient to outpatient).
• Identifies gaps in care and supports program improvements.
