PVH Potomac Valley Hospital in Keyser, WV, seeks a Nursing Care Coordination professional to triage calls, assess patient needs, and facilitate referrals. You will educate patients on immediate care and preventative behaviors while partnering with the care team to design individualized plans.

Responsibilities include documenting conversations, coordinating clinic visits or telehealth, and supporting quality improvement initiatives with data-driven decisions to improve access and cost-effective

Formación

• Current unencumbered RN licensure in WV or applicable state board.
• BLS certification within 30 days of hire date.
• Three years of nursing experience.

Responsabilidades

• Triage incoming patient calls and assess needs with established protocols.
• Provide medical information using nursing background and resources.
• Identify care gaps and intervene to improve outcomes.
• Schedule and coordinate referrals, e-visits, or telehealth visits.
• Educate patients and assist in selecting physicians and facilities.
• Document conversations to maintain a comprehensive medical record.
• Communicate with case management, community health workers, and providers.
• Support clinical performance improvement and data-driven decisions.
