Starting Wage for this exempt position: $102,606.40-$141,107.20, depending on experience
The Nurse Navigator coordinates diagnostic and treatment processes and follows up at assigned Asante facility(s). Guides patients and their families through the continuum of care and collaborative process with medical care providers and ancillary support programs. They act as a central point of contact for all medical care providers working with the patient, and facilitate evidence-based nursing care. They assess educational needs and design, teach, and/or coordinate educational programs for patients, families, and the community.
