Job summary

The Veterans Health Administration, Department of Veterans Affairs, seeks a Registered Nurse to coordinate care focused on patient education, self-management and satisfaction throughout the continuum of care.

Responsibilities include coding the RAI/MDS sections and CAAs, participating in CLC care plan meetings, educating staff on RAI/MDS processes, supporting quality improvement, and contributing to survey readiness within the CLC.

Qualifications

• Requires RN licensure and ongoing nursing practice.
• Experience with RAI/MDS coding and CAAs is preferred.
• Ability to educate staff on RAI/MDS processes and documentation requirements.

Responsibilities

• Coordinate care focused on patient education, self-management, and satisfaction.
• Code RAI/MDS sections and CAAs within CMS/VHA timeframes.
• Participate in CLC care plan meetings and educate staff on RAI/MDS processes.
