Job summary

Fusion Healthcare Rehabilitation and Nursing Center in Gardner, MA is seeking an experienced nurse leader to direct and supervise the resident care program on assigned units. You will ensure safety, infection control, and appropriate care planning, while promoting a respectful, homelike atmosphere for residents.

The role requires graduation from an accredited nursing program and an active RN or LPN license, plus at least one year in long-term care with supervisory experience.

Qualifications

• Must be a graduate of an accredited nursing program and licensed RN/LPN.
• Minimum one-year experience in long-term care; supervisory experience preferred.

Responsibilities

• Direct and supervise the implementation of the resident care plan; ensure safety devices, positioning, toileting, and infection control.
• Coordinate and supervise services on assigned units to create a homelike atmosphere for residents.
• Perform regular rounds on nursing units; check residents who are acutely ill or dying; greet new admissions.
• Ensure staff treat residents with dignity and respect.
• Ensure staff follow facility policies and procedures.
• Provide education and direction for nurses; assist with assessment and care planning during condition changes.
• Monitor incidents and complaints; document correctly; help determine root causes and preventive measures.
• Facilitate communication between nurses and providers; ensure resident care needs are met.
• Assist in staffing decisions and discipline per HR policies; supervise equipment use and supply needs.
