Qualifications

• Graduation from an accredited Nursing School.
• Current and valid LVN/LPN license in the applicable State (Texas).
• Minimum of one-year supervisory experience in a long-term or health care setting preferred.
• Ability to read, write, and speak English.

Responsibilities

• Monitor and review daily records for accuracy, documentation and intervention.
• Make rounds on all patients at a minimum of every 4 hours and as needed.
• Review 24-hour report book and assess changes in patient condition.
• Review medical records on discharge and ensure proper documentation.
• Distribute monthly MDS/care plan calendars and attend interdisciplinary meetings.
• Enter physician orders daily into the clinical software and verify accuracy.
• Attend weekly care plan and Medicare meetings with the interdisciplinary team.
