This role oversees team-based care, care coordination, quality improvement, and staff management while collaborating with providers to improve patient outcomes. The Nurse Manager will coordinate care for complex patients, support disease management and preventive care, conduct assessments, track referrals, and engage families in planning.

Qualifications

• Registered Nurse License in MA is required.
• 1-2 years of clinical experience or equivalent.
• Current CPR certification.

Responsabilités

• Coordinate and manage care for complex patients with PCP and care team.
• Support acute and chronic disease management, preventive care, patient education, and healthy lifestyle goals.
• Conduct comprehensive assessments and communicate patient needs, care plans, and status changes.
• Track referrals and coordinate care across providers and settings.
• Engage patients and families in care planning and self-management.
• Connect patients with Health Center and community resources for access to care.
• Monitor, evaluate, and document patient progress and risk status.
• Collaborate with multidisciplinary teams for high-quality, coordinated care.
• Review patient lists and coordinate complex cases with ACO, Behavioral Health, and Medical care managers.
• Follow the C3 short-term Complex Care Management model and monitor KPIs.
• Facilitate group-based medical visits to promote education and peer support.
