You will oversee RN Case Managers and Case Management Liaisons across four hospital campuses, reporting to the Director of Care Coordination. The role focuses on improving patient flow, length of stay, and discharge planning, using data benchmarking to drive sustainable change, team development, and strong cross‑functional

Qualifications

• BSN required; Master’s degree in Nursing, Management, or a related field required.
• Active OR RN license required, or ability to obtain OR RN licensure.
• ACM certification through ACMA preferred.
• A minimum of five years of clinical nursing experience required, coupled with at least one year of leadership experience.
• Three or more years of leadership experience within Utilization Review, Case Management, or Care Coordination in an acute care setting is highly preferred.

Responsibilities

• Lead systemwide care coordination across four hospital campuses.
• Oversee RN Case Managers and Case Management Liaisons; partner with clinical and operational leaders.
• Optimize care coordination operations to improve patient flow, length of stay, and discharge planning.
• Travel to the system's northern campuses to maintain leadership presence.
