Job summary

Edgewood Manor Rehabilitation and Healthcare Center_ in Port Clinton, OH is seeking compassionate LPNs and RNs to join our patient-centered care team. You will assess, monitor, and support residents' health needs, administer medications, and collaborate with an interdisciplinary team.

Qualifications

Current LPN license in Ohio or RN license in Ohio.
Completion of an accredited nursing program (ADN/BSN).

Responsibilities

Assist in assessment, planning, and evaluation of residents’ conditions; RNs will perform and oversee comprehensive assessments.
Administer medications and treatments as prescribed, monitoring for side effects or complications.
Collaborate with the nursing team to develop and implement individualized care plans.
Provide essential nursing care, including wound care, catheter care, and assistance with activities of daily living.
Educate residents and families on health management, wellness practices, and discharge planning as appropriate.
Maintain accurate and timely documentation of care and resident progress.
Uphold infection control standards and maintain a safe, clean environment.
Participate in staff meetings, ongoing training, and quality improvement initiatives.
RNs may supervise LPNs, STNAs, and other team members as needed.
