Franklin Wellness & Rehabilitation Center in Franklin, TN seeks a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our care team. You will oversee daily CNA activities, participate in admissions, discharges, and transfers, and administer medications per physician orders.

Responsibilities

• Direct the daily activities of CNAs and ensure policies and procedures are followed.
• Participate in admissions, discharges and transfers of residents as required.
• Assess residents' conditions, prepare and administer medications per physician orders.
• Initiate, review, and update care plans.
• Report changes in a resident’s condition to the nurse supervisor and communicate factors affecting work.
• Perform other duties assigned by clinical or administrative leadership.
• Maintain confidentiality of protected health information.

Nurse Position Summary

As a Nurse (LPN or RN) at Franklin Wellness & Rehab, you'll be an essential part of our residents' care. Responsibilities include:

• Direct the daily activities of the CNAs and ensure policies and procedures are followed by nursing personnel.
• Participate in admissions, discharges and transfers of residents as required.
• Assess resident’s conditions, prepare, and administer medications per physician orders.
• Initiate, review, and update care plans.
• Report any changes in a resident’s condition to the nurse supervisor and keep them informed of factors that interfere with being able to perform work as assigned.
• Perform other assigned duties given by clinical or administrative leadership.
• Maintain confidentiality of protected health information, including verbal, written and electronic communications.

Nurse Requirements

• Current unrestricted LPN or RN License for the state of Tennessee
• Current CPR/BLS Certification
