Qualifications

• Baccalaureate degree in nursing preferred.
• Current Massachusetts nursing license.
• Current BLS certification.

Responsibilities

• Oversee nursing care for participants within the program.
• Assess patients by history, tests, and IDT consultation.
• Teach patients and families about disease management and wellness.
• Safely administer medications and manage refills with provider authorization.
• Coordinate health teaching to maintain optimal health.
• Complete biannual nursing assessments and document care plans.
• Use assessment tools to collect data for care planning.
