Job summary

NeighborHealth is seeking a Nurse for the Neighborhood PACE program in Winthrop. You will join an interdisciplinary team in an innovative care model aimed at helping older adults stay healthy and independent within a community health setting.

The role requires a nursing degree, MA license, and BLS, with experience in ambulatory or elder care preferred. Primary location is Winthrop, MA, with a full-time schedule and competitive starting pay.

Qualifications

• BLS certification required; ambulatory/home care experience preferred.
• Experience working in an interdisciplinary team setting is a plus.
• Strong patient education and health teaching skills.
• Care planning and assessment experience essential; nursing assessments performed.
• Medication administration and refills managed with provider authorization.

Responsibilities

• Oversee nursing care for participants; assess and plan care.
• Gather history, perform tests, observe, consult with IDT.
• Educate patients and families on disease, wellness, medications, and plan of care.
• Safely administer medications and manage refills per orders.
• Provide health teaching to individuals, families, and groups to maintain health.
• Complete biannual/status nursing assessments; document care plans.
