Nurse-Family Partnership (NFP) is a national evidence-based nurse home visitation model that pairs nurses with low-income first-time mothers, beginning in pregnancy and continuing until the child's 2nd birthday. The NFP nurse home visitor position offers a rewarding experience for nursing professionals interested in improving health and social outcomes for first-time mothers and babies in Central Brooklyn. NFP nurses are highly supported, valued, and respected within the program and community, with a good work/life balance due to autonomy, flexible schedule, and PTO.

Responsibilities include:

  • Develop and maintain a caseload of 21 to 25 families during the first year.
  • Deliver culturally sensitive services to a diverse population.
  • Maintain fidelity to the NFP model regarding frequency, content, and documentation of services.
  • Adhere to program and agency policies and procedures.

Qualifications:

  • BA in Nursing required.
  • Current RN license (in good standing) required.
  • Minimum of two years recent experience in maternal/child health, pediatrics, or community health required.
  • Excellent written and verbal communication skills.
  • Basic computer skills.
  • Spanish, French, and Creole language skills are a plus.
