Job summary

CentralCare Incorporated seeks a highly skilled Registered Nurse - Disease Management for full-time service at Joint Base San Antonio (JBSA-Randolph). The clinician will oversee care for patients with chronic diseases, perform health assessments, and implement comprehensive disease management plans within a military healthcare setting.

This position requires a BSN, BLS certification, and at least four years of nursing experience in chronic/complex conditions, with a focus on education, care

Qualifications

• BSN from an NLNAC/CCNE-accredited program.
• BLS certification required.
• Minimum 4 years of nursing experience with chronic/complex conditions after BSN.
• Experience in disease management, care coordination, and population health initiatives.

Responsibilities

• Coordinate and manage care for patients with chronic diseases and complex health conditions.
• Conduct patient assessments, health screenings, and telephonic evaluations.
• Develop, implement, and monitor disease management and patient care plans.
• Educate patients, families, and healthcare staff on disease prevention, treatment, and wellness programs.
• Collaborate with physicians, case managers, and interdisciplinary teams to improve patient outcomes.
• Track patient progress, analyze population health data, and support quality improvement initiatives.
• Maintain accurate documentation, reports, and compliance with military treatment facility policies and clinical guidelines.
