Job summary

Spectrum Healthcare Resources has an opportunity for a Registered Nurse Disease Manager at Joint Base Andrews in the Washington, DC area. The role focuses on outpatient clinical care coordination for patients with prevalent chronic diseases such as diabetes, hypertension, and hyperlipidemia.

Candidates must have an Associate Degree in Nursing, an active RN license, at least three years of outpatient nursing experience, and disease-management certifications (CCP, CHC, CDE, or AE-C).

Qualifications

• Active RN license required.
• Minimum 3 years of full-time outpatient nursing experience.
• 2+ years in management/education of patient populations with chronic diseases (diabetes, hypertension, hyperlipidemia).
• Current disease-management certifications such as CCP, CHC, CDE or AE-C are required or strongly preferred.

Requirements

• Associates Degree of Nursing
• Current, full, active, and unrestricted license to practice as a Registered Nurse (Any State)
• A minimum of 3 years of full-time nursing experience in the outpatient clinical setting is required. Two years of experience in the management and education of patient populations with prevalent and chronic diseases to include, but not limited to: diabetes, hyperlipidemia, and hypertension is required.
• Possess current certification in disease-management related area such as:
• Chronic Care Professional (CCP) by Health Sciences Institute
• Certified Health Coach (CHC) by National Society of Health Coaches
• Certified Diabetes Educator (CDE) by National Certification Board for Diabetes Educators
• Certified Asthma Educator (AE-C) by National Asthma Educator Certification Board
• Highly organized, self-directed worker able to function in a high-volume environment without distractions.
• Strong verbal and written communication skills.
• Proficient level of experience with Microsoft Office applications and strong technical aptitude.

Work Schedule

• Monday - Friday
• No nights and weekends
• 8 hours a day
