Job summary

CentralCare Incorporated is seeking a highly skilled Registered Nurse - Disease Management to provide full-time services at Joint Base San Antonio (JBSA-Lackland), supporting military families as federal civilian contractors. The role combines care coordination for chronic conditions with patient assessments, education, and population health initiatives.

Qualifications

• BSN from NLNAC/CCNE-accredited program.
• BLS certification required.
• Minimum 4 years of nursing experience managing chronic/complex conditions.

Responsibilities

• Coordinate and manage care for patients with chronic diseases and complex health conditions.
• Conduct patient assessments, health screenings, and telephonic evaluations.
• Develop, implement, and monitor disease management and patient care plans.
• Educate patients, families, and staff on disease prevention, treatment, and wellness programs.
• Collaborate with physicians, case managers, and interdisciplinary teams to improve patient outcomes.
• Track patient progress, analyze population health data, and support quality improvement initiatives.
• Maintain accurate documentation, reports, and compliance with military treatment facility policies and clinical guidelines.
