Job description

As the Clinical Manager, you will work one-on-one with clients to create customized care that will be delivered by your nursing and aide staff. With your oversight and training, clients will receive excellent care in the comfort of their home. You will make a meaningful difference daily while working alongside a supportive team. Responsibilities

1. Perform case management, including initial intake, managing authorizations, communicating with referral sources, oversight, and care coordination for all disciplines.
2. Supervise daily clinical and office operations.
3. Provide clinical supervision of patient care according to nursing principles and agency policies. Train and mentor field clinicians and aides.
4. Ensure services comply with professional standards and regulatory requirements.

Qualifications

1. Current RN license in the state of Florida.
2. At least one year of home care experience preferred.
3. Ability to travel to clients' homes as needed.
4. Reliable transportation and a valid driver’s license.
5. Excellent time management skills.
6. Positive attitude and strong work ethic.
7. Excellent communication and customer service skills.
8. Experience with Matrix/AlayaCare is a plus.

Pay range: $67,000 - $73,000 per year.
