We are seeking a Field Nurse Case Manager to cover the San Jose, CA, and surrounding areas. This is a home-based role involving travel up to 2 hours one-way to various locations such as doctors’ offices and hospitals. The individual will be responsible for medical case management of work-related injuries, including assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager will collaborate with insurance carriers, medical providers, attorneys, employers, and employees, monitoring progress and reporting results.

Qualifications:

• Current, unencumbered RN license in California
• Experience in workers’ compensation case management preferred
• Certifications such as CCM, CRRN, CDMS, CRC are strongly preferred; must be obtained within two years if not already held
• Skills: organization, technology proficiency, communication (written, verbal, interpersonal)
• Ability to work independently with minimal oversight
• Patient education skills
• Valid driver’s license with a good driving record
