At Cypress Independent Living Services, Inc., we empower individuals by providing compassionate and effective life skills coaching and case management services. As a vital part of our mission, you’ll be the first point of contact for clients and their families, guiding them toward success and independence.

What You’ll Do:

1. Conduct intake assessments to evaluate client needs and goals.
2. Develop and oversee individualized care plans tailored to each client’s unique situation.
3. Serve as the main point of contact for clients, families, and service providers, ensuring seamless communication and support.
4. Monitor and document client progress, updating plans as needed.
5. Advocate for clients by connecting them to appropriate resources and services.

What We’re Looking For:

1. At least 2 years of direct intake and/or case management experience, preferably working with developmentally disabled individuals.
2. Strong interpersonal and communication skills, with the ability to build trust and rapport.
3. Exceptional organizational skills and attention to detail.
4. A passion for helping others and a commitment to making a difference.
5. Reliable transportation and willingness to travel throughout the San Jose area.
