Qualifications

• Current, unrestricted Rhode Island RN license.
• Graduate of an accredited nursing program.
• Current CPR certification.
• Strong clinical assessment, nursing judgment, communication, and organizational skills.
• Ability to independently manage multiple individuals with varying healthcare needs.
• Ability to communicate effectively with individuals with IDD, families, staff, and healthcare professionals.
• Strong documentation and electronic recordkeeping skills.

Responsibilities

• Provide nursing case management for an assigned caseload in residential settings.
• Review medical histories, diagnoses, medications, treatment plans, allergies, and risks.
• Develop, maintain, and update nursing assessments, healthcare plans, and protocols.
• Monitor health conditions and communicate changes to healthcare providers and team.
• Coordinate follow-up care after ER visits, hospitalizations, or changes in medical status.
• Participate in discharge planning and transitions of care.
• Promote preventive healthcare and ensure screenings and immunizations are addressed.
• Coordinate medical appointments, labs, and diagnostic testing.
• Communicate with PCPs, specialists, pharmacies, hospitals, families, and guardians.
• Review orders and ensure proper communication and implementation.
• Advocate for individuals to ensure continuity of healthcare services.
• Provide education on diagnoses, protocols, medications, and signs requiring intervention.
• Develop health-related training for residential staff.
• Maintain nursing documentation and support regulatory readiness.
