Job summary

VALIR PACE LLC is seeking a PACE Registered Nurse to deliver integrated clinical care, coordinate services, and perform intake assessments for participants and potential enrollees. This role operates within the interdisciplinary team to support health outcomes across clinic, home, and community settings.

The RN will ensure compliance with CMS and PACE regulations while providing participant-centered nursing care throughout enrollment.

Qualifications

• Current RN license in good standing (state-specific).
• Current CPR Certification.
• Valid Driver’s License for field-based work.
• ACLS and UCAT certifications preferred (as applicable).
• Geriatric, home health, or PACE experience preferred.
• Prior in-home assessments and care coordination experience strongly preferred.

Responsibilities

• Provide comprehensive clinical nursing care and care coordination for PACE participants and enrollees.
• Perform intake assessments and participate in IDT care planning.
• Deliver nursing services across clinic, home, and community settings.
• Ensure compliance with CMS, PACE regulations, and participant-centered care.

This role functions as an integrated member of the Interdisciplinary Team (IDT), supporting participant health outcomes through assessment, care planning, coordination, education, and hands-on nursing services across clinic, home, and community settings.The RN ensures compliance with federal, state, and CMS PACE regulations while delivering participant-centered, high-quality nursing care throughout the participant lifecycle—from intake through ongoing enrollment.Education, Licenses, Certifications and Experience:Current Registered Nurse (RN) license in good standing (state-specific).Current CPR Certification.Valid Driver’s License (for field-based work).ACLS and UCAT certifications preferred (as applicable).Experience in geriatric, home health, or PACE preferred.Prior experience with in-home assessments and care coordination strongly preferred.
